Confidentiality and access to records

The practice complies with data protection and access to health records legislation. Identifiable information about you is shared with others only to provide further medical treatment, for example with district nurses or hospital services; to help you get other services such as social care, which requires your consent; or where we have a duty to others, for example in child protection cases.

Anonymised patient information is also used at local and national level to help plan services, for example for diabetes care. Please tell us if you do not wish anonymous information about you to be used in this way.

Reception and administration staff need access to your medical records to do their jobs, and are bound by exactly the same rules of confidentiality as clinical staff.

You can view your record through the NHS App or your online account. Proxy access lets a named person, such as a carer or parent, act on your behalf where the appropriate consent is in place.

You may request a copy of your health record. Requests should be made to the practice manager and no information is released without your consent unless we are legally obliged to do so. Information about the GPs and the practice that must be disclosed under the Freedom of Information Act is also available on request to the practice manager.
