You do not usually need a fit note for the first seven calendar days of illness. Your employer can accept a self-certification for that period.
Request a fit note, or an extension to an existing fit note, online. Tell us the dates you need covered and the reason for your absence.
Fit notes for the first seven days, and notes requested purely for an employer's own paperwork, are not an NHS service and may attract a charge. See our page on non-NHS services and fees.
