Referral information

How referrals are made and how to follow up an existing referral.

All patient information

Referrals are made by a clinician after an assessment. Once a referral has been sent, the receiving service is responsible for arranging your appointment and will contact you directly.

Under NHS Right to Choose you can ask to be referred to any NHS-commissioned provider in England. Tell the clinician if you have a preference and we will support your choice.

To follow up an existing referral, or to provide information the practice has asked for, contact us online.
