Shared-care medicines

Shared care is an arrangement where a specialist and your GP work together to support your ongoing treatment. This may include prescribing medication, monitoring your health, and reviewing your treatment in line with an agreed care plan.

Historically, our ability to accept new shared care arrangements has been limited due to national GP collective action and local commissioning arrangements.

All shared care requests are considered on an individual basis. Acceptance will depend on factors including whether there is an appropriate shared care agreement in place, the specialist's ongoing involvement, local commissioning arrangements, and whether it is clinically safe and appropriate for the practice to participate.
